This is very well done! The only suggestion I have is maybe a small penalty for missing items. If there is one, I was very fortunate not to deal with it :-)
I think the hit reaction boxes of the bad items is too big. I keep losing health when I catch all the food and avoid all the bad stuff, I've lost games without actually colliding with any pencils, bats, fire hydrants, apples, or whatever the gray thing is

I just fixed a bug where the iron nose power-up (gray thing) caused the player to take damage on de-spawn. Thank you for your feedback!
